All participants (students, teachers, accompanying parents or adult volunteers, and accompanying minors) are required to complete the appropriate forms found below.

STEP 1: Complete the Application Form. Fees and covered expenses are also listed on this form.

STEP 2: Access the appropriate waiver form(s) below and complete them.

for STUDENTS   --   for TEACHERS, PARENTS, and ACCOMPANYING ADULTS   --   for ACCOMPANYING MINORS

STEP 3: Provide all required supplemental material.
(a) Copy of passport and insurance card (front & back)
(b) Include a 2'' x 2'' headshot photo
